Default what happens if a quit claim is filed wrong in ohio?

signed quit claims over to ex after divorce. wording has him as married on the claim. Now he wants me to sign another one so he can get a loan. I was told it was filed wrong with the wording listing him as married. Why would another have to be signed again?
