Default Quit Claim, Property transfer

My husband and I purchased a property several years ago and we had my mother on the title. As she became older she wanted to make sure we would not have any legal issues after she passed away and we completed a quit claim deed that was signed and notarized. The document was never recorded. My mother passed away a couple of years ago and this year we were going to get an equity loan and since her name was on the title, they needed her signature. We explained she passed away and that we have a quit claim deed but they say we needed to fix the title first before they could process the equity loan. Where should I go to have the title changed?
